II. Permission and Release 

A. The Culinary Education Program and ACF of Milwaukee Agreement & Waiver: 

I understand that my child will be in an industrial kitchen that does not have the same safeguards as a home kitchen. He/she will be around stoves, ovens, burners, and sharp equipment such as knives. I agree that The Culinary Education Program, ACF of Milwaukee, its Officers, Board of Directors, Administrators, employees, and agents and staff are not responsible, nor liable, for accidents, loss, damage, death, delay or expense arising from acts of God, terrorism, wars, strikes, quarantine, weather irregularity, equipment failure, vehicle accidents, government restrictions or regulations, nor is responsible for illness from food or otherwise, detention, assault, theft or criminal activity or annoyance. I agree to release and hold harmless The Culinary Education Program and ACF of Milwaukee, it’s officers, trustees, agents and employees, and agree to indemnify each of them from any and all claims, cost suits, actions, judgments and expenses, upon any damage, loss or injury to my child or damage to my child’s property arising out of my child’s participation in this activity. 

B. Late Pick-Up: 

Camp ends at 4:00pm for Youth & 2:00pm for Kids each day. Please understand that when a parent is late picking up their child, we must have at least two instructors remain at the site for the safety of your child. These instructors must be compensated for their time and this can ultimately cause an increase in our program fee. We have a 20-minute grace time. Fees apply beginning at 4:30pm for Youth or 2:30 for Kids.

C. Photo Release:

Several outside publications and Television Networks approach The Culinary Education Program or ACF of Milwaukee for last minute stories and publications. I hereby give my permission for my child’s picture to be used by The Culinary Education Program or ACF of Milwaukee publications or video, and Third-Party publications running stories in publication or video or for all marketing purposes.

D. Authorization of Treatment: 

I hereby give my permission to the staff of The Culinary Education Program or ACF of Milwaukee for treatment of my child in minor cuts or burns. In the case of a serious injury where I deem it necessary for emergency treatment and I cannot be reached, I give my permission for transport and treatment at a local hospital at my expense.

E. Camp Cancellation Policy: 

Any camp cancellations made at least one month prior to the camp start date will receive a refund minus a $25 cancellation fee. NO refunds given after June 30, 2023

F. Camper Code of Conduct:

In order to maintain a safe and fun environment The Culinary Education Program or ACF of Milwaukee requires parents and campers to read and understand the importance of following the code of conduct.

DRESS CODE: Campers should be dressed both comfortably and safely in case of food and heat contact. Because the kitchen can be a dangerous place:

  • Campers must wear closed toed comfortable shoes (i.e., clogs, crocs, tennis shoes) to protect their feet. Please no flip-flops, sandals or ballet shoes, etc. 

  • No Tank Tops with drawstrings or shirts with loose sleeves. Please wear sleeves. 

  • No Jewelry

  • Campers will be given a culinary kit that is theirs to keep; however, the kit should remain at the campsite until the last day of camp. White apron and hat (provided by camp)

  •  T-Shirts may be provided to campers and must be worn the day of field trip if any. 

  • NO CELL PHONES, IPODS, GAMES, etc. in the kitchens. 

  • Campers must show respect towards all others while at camp; this includes other campers, volunteers and instructors. BULLYING WILL NOT BE TOLERATED AND IT IS GROUNDS FOR DISMISSAL!

A Note on Camper Conduct: The Culinary Education Program or ACF of Milwaukee reserves the right to dismiss a camper at any time because of a violation of camp rules or because of disruptive personal conduct which interferes with the safety or welfare of the individual or others. No refunds will be made.
